We are excited you are interested in applying to serve on a USTA Florida Commitee. Please read the requirements below to be eligible to apply. 

To be eligible for consideration applicants are required to:

• Be a Florida resident and Florida be your primary residence. 
• Be USTA Safe Play approved. You will be required to upload a copy of a valid USTA Safe Play Certificate to be considered. Click here for info on how to get a copy of your certificate.

If you meet these eligibility requirements, click the "Submit" button to begin. Keep in mind that you will be able to save a draft version of your application at a later time if need be.
